Job Description:

Support primary care providers with clinical documentation; focus on accurate and specific documentation; assist providers throughout the patient care journey; document patient encounters in real-time; prepare after visit summaries; identify and close care gaps; extensive on-the-job training provided in clinical workflows and value-based care; strong focus on team-based care.

Job Responsibility:

Documenting patient encounters
joining the provider in the exam room
preparing after visit summaries
assigning appropriate CPT and ICD-10 codes
reviewing medical records
leveraging population health tools
supporting daily huddles
consulting with provider on clinical documentation
administrative support such as placing orders and referrals
addressing tasks

Requirements:

Knowledge of medical terminology and common medications
prior clinical experience
prior scribe or transcription experience preferred but not required
advanced listening and communication skills
strong computer literacy
fluency in Spanish, Polish, Russian, or other languages spoken by people in the communities served for specific roles
ability to adapt to new workflows
ability to type 70+ words per minute
ability to work in-person in clinics
commitment to at least 1 year in the role
ability to work approximately 40-45 hours per week during clinic hours
compliance with hospital and Oak Street Health policies
US work authorization
