Core responsibilities universally include conducting comprehensive nursing assessments to determine hospice eligibility, developing and continually updating individualized plans of care, and making regular visits to monitor symptoms, manage pain, and address evolving needs. A critical function is serving as the communication hub of the interdisciplinary team (IDT), which includes physicians, social workers, chaplains, and aides. The RN Case Manager coordinates all services, initiates necessary interventions, and ensures every aspect of the patient’s well-being—physical, psychological, social, and spiritual—is addressed. They are also responsible for the clinical pronouncement of death and providing immediate, sensitive support to the grieving family. Meticulous documentation, adherence to regulatory standards, and supervision of home health aide services are fundamental to the role. Typical requirements for these positions include graduation from an accredited nursing program and an active, unencumbered Registered Nurse (RN) license in the state of practice. Current CPR and often First Aid certification are standard. Given the community-based nature of the work, a valid driver’s license and reliable transportation are almost universally required.
