They perform medicines reconciliation, a critical process of creating the most accurate list of a patient's medications when they are admitted to or discharged from a hospital. A significant part of their day is spent providing evidence-based advice and recommendations to doctors, nurses, and other prescribers on drug selection, dosing, and monitoring. They are often responsible for managing therapy for patients with complex, long-term conditions, such as diabetes, hypertension, or asthma, and play a vital role in antimicrobial stewardship to combat antibiotic resistance. Their work ensures that pharmacotherapy is tailored to the individual patient, maximizing benefits while minimizing risks. To succeed in Clinical Pharmacist jobs, specific skills and qualifications are universally required. A foundational requirement is a Master of Pharmacy (MPharm) degree or equivalent, followed by completion of a pre-registration year and mandatory registration with the relevant national regulatory body, such as the General Pharmaceutical Council (GPhC) in the UK.
