Hemtjänstkoordinator

• Plan and schedule ordered interventions from care assessors and healthcare personnel
• Work towards set goals and strategies
• Close collaboration with unit managers in home care and several other professional groups
• Optimize scheduling in a resource-efficient way, including finding logical solutions for intervention planning with the user in focus
• Daily communication with SÄS to monitor users' discharge and return home
• Manage staffing during short-term absences and replan intervention schedules when necessary
• Work in systems such as TES, SAMSA, Time Care pool, VIVA, etc.
• Daily use of Excel, Word, and communication via email and telephone

• After-secondary education in administration or at least one year of documented experience in planning home care services
• Good analytical and logistical ability
• Commitment and willingness to develop and improve planning based on set goals
• Good communication skills in both speech and writing
• Good collaboration skills and high service orientation
• Flexibility to handle rapid changes and multiple tasks simultaneously
• Good computer skills and ability to handle various business systems and the Office package
• Understanding of the relationship between the different parts governing home care (Social Services Act and Health and Medical Services Act)
• Understanding of the relationship between the unit's needs, staff, and economy
• Professionalism and ability to make decisions based on your responsibility role
• B driver's license is a requirement for the position

Experience working as an assistant nurse in home care is meritorious
