Hemtjänstkoordinator

• Plan and schedule ordered interventions from care managers and healthcare personnel
• Work towards set goals and strategies
• Close collaboration with unit managers in home care and several other professional groups
• Optimize scheduling in a resource-efficient way
• Daily communication with SÄS (likely a healthcare authority) to monitor clients' discharge and return home
• Manage staffing for short-term absences and replan intervention schedules when needed
• Work in systems such as TES, SAMSA, Time Care pool, VIVA, etc.
• Daily use of Excel, Word, and communication via email and telephone

• After-secondary education in administration or at least one year of documented experience in planning home care services
• Good analytical and logistical ability
• Good communicative ability in both speech and writing
• Good cooperation skills and high service orientation
• Flexibility in working methods
• Ability to handle multiple tasks simultaneously
• Good computer knowledge and ability to handle various business systems and the Office package
• Understanding of the relationship between the different parts governing home care (Social Services Act and Health and Medical Services Act)
• Understanding of the relationship between the unit's needs, staff, and economy
• Professionalism and ability to make decisions based on the responsibility role
• B driver's license is a requirement for the position

Experience working as an assistant nurse in home care is meritorious
