Providing high quality accommodation, outreach and support services for young people living across West Sussex and Brighton & Hove. The team supports professionals working with young people aged 16-24, focusing on their wellbeing, welfare and skills development to aid successful transition into adulthood and independent living. We also offer Independent Visitor Services and Specialist Services, including CSE (child sexual exploitation) and particular expertise with UASC (unaccompanied asylum seeking children). We are committed to making a positive difference to young people who are facing a period of change, uncertainty or are in need of support from a significant adult. As Outreach Worker, you will be working with vulnerable young people, supporting them with independent living skills development. You will be required to safeguard and support the young people with key work sessions, be a guardian of the property, facilitate the house clean at all times, support the young people with their day to day routine, support the young people to adhere to the house rules and enabling the young person’s to support their independent and individual lifestyles.
Job Responsibility:
Deliver care support to all young people being supported by care team
Provide outreach support to young people referred by social services
Adhere to outcome targets set by the Service Manager
Contribute to the provision of homes for marginalised young people
Promote harmonious living within the homes
Report maintenance issues
Encourage and equip the young people through ILS training programme
Promote independence via information, advice and guidance
Assist in overcoming any barriers service users may face
Maintain a caseload of individual referrals
Accompany service users to meetings as appropriate
Produce reports for social services
Participate in the collection of service users feedback
Work with other professionals to ensure timely and appropriate progression into independent living accommodation
Uphold staff protocols
Maintain/produce data reports for internal and external purposes
Maintain data management of all information regarding services users
Deliver recording of information and activities
Represent and set a good example
Observing equal opportunities policy
Implement all policies and procedures in relation to caring for young people
Maintain regular and consistent contact with young people
Monitor attendance, punctuality and engagement of service users
Liaise with other staff in other locations
Fully engage and participate in team meetings
Seek to improve the service provision
Be proactive and innovative in devising new methods of working with service users
Develop relationships with other professionals
To fully participate in creative meetings and discussions
Identify resources to meet the needs of the service users
Initiate, develop, produce new document practises

Implement financial policy and practise
Engage in dialogue and action in regards to good financial management
Be economical with resources
Ensure all relevant and appropriate expenses are authorised
Maintain information and records of all work undertaken for audit purposes
Liaise with other professionals regarding reimbursements
To be responsible for any required reporting
Manage Petty Cash and receipts system
Complete and submit all receipts on a weekly basis
Participate in managing a house keeping budget
Requirements:
Qualification related to youth work or health care (desirable)
Experience maintaining accurate records, paying attention to detail and meeting deadlines
Experience of working with young people
