The Fox Hill Village Health & Wellness Center is a Nurse Led model that provides health, wellness, and urgent care type services to maximize the health and well-being of all residents of Fox Hill Village. The Health & Wellness Center Registered Nurse is responsible for the assessment, planning, intervention, and evaluation of nursing care for individuals, families, and the community.
Job Responsibility:
Works collaboratively with Fox Hill Village health professionals to provide optimum nursing services to the residents of Fox Hill Village
Demonstrates the competencies of the gerontological registered nurse as described in the Standards of Gerontological Nursing Practice
Utilizes a holistic approach in providing nursing services to residents
Develops individualized wellness plans and goals with individual residents to maximize health and well-being and help residents attain their highest level of functioning
Uses Evidence-Based Practice methods in providing care to residents
Utilizes comprehensive assessment skills in triage of residents seeking care for health problems, minor illnesses and injuries and in performing wellness assessments
Includes family members when requested by residents or when otherwise appropriate
Implements treatments and other elements of care prescribed by a physician’s order or by the primary care provider
Maintains competency in Point of Care (CLIA-waived) Testing conducted in the Health & Wellness Center
Manages and performs periodic health maintenance/health monitoring activities as requested or needed by residents, such as blood pressure readings, weight measurement, etc
Utilizes RN-assisted tele-health visits between residents and their primary care providers to ensure understanding of the treatment plan, continuity of care and optimal communication
Provides clinical teaching for individual residents/family that facilitates resident learning across the care continuum. Creates a teaching/learning environment that meets residents’ individual learning styles
Addresses ethical and clinical concerns of the resident/family consistent with the Code of Ethics for Nurses
Uses systems thinking to utilize environmental and system resources and identify barriers that exist for the resident at Fox Hill Village and within or across healthcare systems as they pertain to the resident’s care and goals
Accurately documents care provided to residents
Assists in the development, and implementation of standards, care protocols, policies, and procedures for the Fox Hill Village Health & Wellness Center
Participates in planning and presenting health education programs for residents
Collaborates with other Fox Hill Village health and wellness professionals (such as Support Services, Fitness Department and Vitalize 360 Program) to enhance programs for residents
Responsibilities may require performance of tasks that involve potential for exposure to blood, body fluids, or tissues
Requirements:
Holistic approach to nursing practice
Strong assessment, triage, and judgement skills
Demonstrated client and family education skills
Strong interpersonal, written, clinical documentation and oral communication abilities
Strong listening skills, while being approachable and articulate
Demonstrated effective collaboration and organizational skills
Exceptional computer skills, with proficiency with Microsoft Office (Windows, Outlook, Excel)
Ability to work weekends, holidays, and be on call for emergencies
Minimum of a Bachelor’s degree in Nursing
Five years of experience as a Registered Nurse, with two or more years’ experience in Gerontology preferred
Current unrestricted licensure as a Registered Nurse in the Commonwealth of Massachusetts
Certification as a Gerontological Nurse – Board Certified (GERO – BC) by the American Nurses Credentialing Center or other recognized nursing certifying body preferred, or must be willing to obtain Gerontological Nurse certification if not currently certified
Knowledge of nursing theories, principles, and concepts acquired through completion of a graduate program in nursing at an approved school of professional nursing
Current Basic Life Support/Healthcare Provider
RN License (Required)
Nice to have:
Master’s degree in Nursing preferred
Two or more years’ experience in Gerontology preferred
Certification as a Gerontological Nurse – Board Certified (GERO – BC) preferred
