Caring and empathetic attitude with a focus on helping people to maintain independence
Ability to provide a person-centred, flexible and responsive service
Experience of managing own workload, using decision making skills to manage and prioritise
Ability to work well within a team and use own initiative
Excellent communication skills with ability to build relationships with individuals, health care professionals and colleagues
Digital skills to manage time efficiently and effectively, including Microsoft Outlook and Excel, and use of iPad and smartphone devices
Ability to manage a busy workload, keep accurate records and conduct assessments
Able to undertake general physical activity in people’s homes (often over 2 floors) for example providing light domestic duties
Full driving Licence, with appropriate insurance cover, and willingness to transport patients in own car

Job Responsibility

Take patients home directly from the hospital and settle them back at home, ensuring they have everything they need to remain at home
Provide continued support for up to 6 weeks, to help them retain or regain their independence
Use a solutions focused approach to ensure that people can access the help that they need when they need it, either from Age UK Lancashire or from other community-based support
