Job Description

Age UK Lancashire’s Living Well Support Service provides invaluable support to clients over the age of 18, helping them attain or regain independence after a hospital stay or prolonged illness. Our service is integral to supporting hospital discharges, preventing unnecessary hospital admissions, and may reduce the need for residential care.

Job Responsibility

  • Take patients home directly from the hospital and settle them back at home, ensuring they have everything they need to remain at home
  • Provide continued support for up to 6 weeks, to help them retain or regain their independence
  • Use a solutions focused approach to ensure that people can access the help that they need when they need it, either from Age UK Lancashire or from other community-based support

Requirements

  • Caring and empathetic attitude with a focus on helping people to maintain independence
  • Ability to provide a person-centred, flexible and responsive service
  • Experience of managing own workload, using decision making skills to manage and prioritise
  • Ability to work well within a team and use own initiative
  • Excellent communication skills with ability to build relationships with individuals, health care professionals and colleagues
  • Digital skills to manage time efficiently and effectively, including Microsoft Outlook and Excel, and use of iPad and smartphone devices
  • Ability to manage a busy workload, keep accurate records and conduct assessments
  • Able to undertake general physical activity in people’s homes (often over 2 floors) for example providing light domestic duties
  • Full driving Licence, with appropriate insurance cover, and willingness to transport patients in own car
