Job Description:

The service supports people to improve their health and wellbeing, and access wider support that meets their needs. This is a great opportunity to support local communities and work with Unpaid Adult Carers, Young Adult Carers and Young Carers.

Job Responsibility:

Support Unpaid Carers across Lewisham
Complete assessments to understand people’s individual circumstances and level of need
Carry out home visits and community meetings
Co-produce support plans
Develop effective relationships with Unpaid Carers, families, and professionals
Signpost and facilitate access for Unpaid Carers to support services
Support the development of escalation plans

Requirements:

Relevant personal or professional experience of working with Unpaid Carers and/or vulnerable adults or children in a relevant sector
Practical understanding of barriers faced by Unpaid Carers
Awareness of cultural differences and access to services
Excellent communicator
Able to motivate and empower others
Right to Work in the UK

Nice to have:

Experience of working in mental health or dementia services
