Graduate of a state approved school of practical (vocational) nursing and currently licensed in the state(s) in which practicing
Minimum of one (1) year experience in nursing, preferred
Acceptance of philosophy and goals of this Agency
Ability to exercise initiative and independent judgment
Ability to perform the following tasks if necessary: Ability to participate in physical activity
Ability to work for extended period of time while standing and being involved in physical activity

Job Responsibility

Understands and adheres to established policies and procedures
Implements the nursing care plan for each patient
Provides nursing services, treatments and diagnostic and preventive procedures as assigned
Initiates preventive and rehabilitative nursing procedures as appropriate
Observes signs and symptoms and reports to the physician and RN reactions to treatments, including drugs and changes in the patient's physical or emotional condition
Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home
Evaluates with registered nurse the effectiveness of the LPN's nursing service to the patient and family under the guidance of the registered nurse
Maintains accurate and complete records of observations, treatments and care of patient
Participates in medical record audit as assigned
Attends staff meetings, patient care conferences and inservices as scheduled

Job Responsibility

Develop a plan of care through physician orders, client input, and nursing assessment to include medical interventions and measurable goals and outcomes
Educate clients and their family members based on client’s specific needs
Properly orient and train primary caregivers to ensure the most optimal functioning level for each client
Coordinate the continuum of hands on client care through documentation and timely communication with the client's physician and other caregivers
Provide care in patients home using a variety of skills such as phone triage, patient education, observation/assessment, wound care, infusions, catheter care, PICC line dressing changes etc.

Current State Licensee as a Registered Nurse RN

Job Responsibility

Assesses patient needs and obtains data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status and incorporate that data into the plan of care
Conducts the initial evaluation and re-evaluates the patient’s nursing needs during each visit
Makes follow up visits according to patient, family, and facility needs
Communicates significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care
Revises the plan of care in response to identified patient care issues and notifies the Clinical Supervisor, physician, facility, and other team members
Counsels, instructs, and includes the patient, facility, and family in following the Interdisciplinary Plan of Care
Teaches hospice philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted-living facilities, etc.)
Collaborates/Communicates with Medical Director, attending physician, branch leadership and IDG
Performs other duties/projects as assigned
