Job Description:

We are seeking a compassionate and dedicated Support Worker to join our team. The ideal candidate will play a vital role in providing high-quality care and support to individuals in various settings, including home care, nursing homes, and hospice care. This position requires a strong commitment to enhancing the well-being of our clients while promoting their independence and dignity.

Job Responsibility:

Provide personal care and assistance with daily living activities
Support clients with mobility and transfers, ensuring their safety at all times
Assist with medication management as per care plans and guidelines
Engage clients in meaningful activities to promote social interaction and mental stimulation
Implement behaviour management strategies to support clients with challenging behaviours
Maintain accurate records of client progress and report any changes in condition to the nursing staff or care manager
Drive clients to appointments or social activities as required, ensuring their comfort during transport
Collaborate with healthcare professionals to develop and update individual care plans tailored to each client’s needs

Requirements:

Previous experience in care
Certification in First Aid is preferred
additional training in nursing or related fields is an advantage
Strong interpersonal skills with the ability to build rapport with clients and their families
Excellent communication skills, both verbal and written
Ability to work independently as well as part of a team
A valid driving licence is essential for transporting clients when necessary
A genuine passion for providing high-quality support and care to individuals in need
