Requirements

Current NYS RN license and registration
Associates degree required
Bachelor of Nursing degree preferred
Bilingual BSN new graduates encouraged to apply
One year of experience in Homecare, acute care or community health nursing preferred
Working knowledge of Medicare, Medicaid and NYS DOH standards and regulations
Bilingual Spanish, Mandarin, Cantonese, Korean, Creole, Bengali, Hindi, and Arabic desirable

Job Responsibility

Independently perform home visits to conduct functional, psychosocial and environmental safety assessments
Determine eligibility of enrollees
Establish an individual plan of care that enables them to remain safely at home despite long-term care needs

Reviews previous nursing assessments/medical notes to determine necessary adjustments/updates in Care Plan and/or to assist with nursing home admission
Conducts in-person Care Management visits in support of person-centered service planning
Liaisons between the member and the CMT
assesses home environment and psychosocial status
Provides feedback to home care agencies and CMT regarding home care issues and contract nurses and aides performance
Provides grass roots, community-based training for frail population care including self-care techniques and prevention strategies
