Requirements

Be trained and credentialed for all health plan clients and lines of business in primary coverage state(s)
consistently engage with regular manager, team and clinical meetings
be accountable to Signify Health operational, clinical, and member experience key performance indicators and metrics
conduct Video Visits as needed
maintain training, affiliation, and compliance with Video Visit requirements
maintain all required supplies and associated diagnostic equipment related to diagnostic and preventive service (DPS) offerings
maintain active state licenses in coverage areas
maintain board certification (as applicable)
maintain multi-state licenses (preferred)
experience with Signify Health In-Home Health Evaluations, in-home visits, or a similar format (preferred)

Job Responsibility

provide 40 hours of availability weekly
execute a strong visit completion rate
conduct Video Visits as needed
meet people where they are and re-engage them with their health care
perform medication and medical history review
conduct a physical evaluation
perform additional diagnostic tests if ordered by the health plan
