Requirements

Active state licenses in coverage area
board certification
experience with Signify Health In-Home Health Evaluations or similar format (preferred)
ability to travel locally and within licensed states (up to 25%)
ability to sit, stand, bend, reach, push, pull and lift up to 20 pounds of supplies
required training and credentialing for all health plan clients and lines of business

Job Responsibility

Conduct pre-scheduled in-home visits with health plan members
provide at least 12 hours of availability weekly
execute a strong visit completion rate
conduct video visits as needed
maintain required supplies and equipment
complete diagnostic and preventive service products
re-engage members with their healthcare during visits
