Degree in Physical Therapy or Occupational Therapy from an accredited program
Current state licensure as a Physical Therapist and/or Occupational Therapist
Previous clinical or home care experience preferred
Strong understanding of rehabilitation principles and practices
CPR/First Aid certification (as required)
Holds a valid level one Fingerprint card
Ability to travel daily including weekends

Job Responsibility

Conduct comprehensive physical and/or occupational therapy assessments
Develop and implement individualized treatment plans based on client goals
Provide therapeutic exercises, functional training, and rehabilitation techniques
Assist clients in improving mobility, strength, coordination, and daily living skills
Train clients in the use of adaptive equipment and assistive devices
Educate clients and caregivers on therapy plans, exercises, and safety practices
Monitor and document client progress and modify treatment plans as needed
Collaborate with physicians, nurses, caregivers, and other healthcare professionals
Maintain accurate and timely clinical documentation in compliance with regulations
Promote client independence and safety in home or care settings
