Transportation Escort. At Blue Ridge Care, we deliver extraordinary care to improve life’s journey. As a mission-driven, not-for-profit healthcare organization, we serve our communities through integrated hospice, serious illness care, PACE services, community thrift shops, and grief support. Our teams meet every participant and family with compassion, expertise, and respect. We are looking for a Transportation Escort to join our PACE team. This role supports participants by providing safe, reliable transportation and escort services between their homes, the Center, and medical appointments. This work is essential in helping participants maintain independence and access the care they need. The Transportation Escort assists with daily transportation coordination and provides hands-on support for participants during transport. This includes planning routes, communicating with the interdisciplinary team, and ensuring a safe environment inside and outside the vehicle. You will help participants board and exit vehicles, maintain required documentation, and ensure all transportation safety standards are met.
Job Responsibility:
Assist with planning daily transportation routes within the PACE service area
Help coordinate efficient use of PACE vehicles
Transport participants to and from the Center based on their scheduled attendance
Provide safe transport in accordance with state DMV regulations and local laws
Escort participants to and from medical appointments as assigned by the care plan
Provide physical assistance for participants entering and exiting vehicles
Ensure emergency medical information is available during transport
Confirm that safety belts are used and wheelchairs are locked into place
Maintain a stocked and compliant first-aid kit
Keep the vehicle clean, organized, and ready for daily use
Monitor for maintenance needs and coordinate routine and episodic service
Maintain accurate logs including number of trips, participants transported, mileage, and fuel usage
Report any mechanical issues, incidents, or safety concerns to the Center Supervisor immediately
Perform related duties as assigned to support daily Center operations
Requirements:
High school diploma required
Current CPR certification
ServSafe certification preferred (may be obtained after hire)
At least one year of experience working with frail or elderly adults
Must pass a Background Check, Motor Vehicle Record review, and Drug Screen
Valid driver’s license, proof of insurance, and reliable transportation
Medical clearance for communicable diseases and up-to-date immunizations
Ability to read, write, and speak English
Ability to work with individuals from diverse backgrounds
Strong communication skills and the ability to remain calm in emergencies
Knowledge of basic safety and infection-control practices
Nice to have:
ServSafe certification preferred (may be obtained after hire)
