Job Responsibility

Coordinates the multidisciplinary screening and evaluation of patients seen in the transplant practice at MSH and approved satellites
Ensures all necessary lab tests are performed on potential transplant candidates and that results are updated as required
Participates in making recommendations regarding placing as patient on the transplant waiting list
Educates patients on the benefits of living donation (when applicable)
Communicates with all active transplant candidates regarding waiting list status on an ongoing basis
On call responsibilities include facilitating all organ offers from UNOS
Follows patients during their hospitalization and coordinates with transplant team
Educates patients and family regarding all transplant medications, lifestyle changes, as well as outpatient follow-up protocols
Assesses continued recovery of patient through compliance with outpatient visits, scheduling of blood tests, regular review of labs, adjusts immunosuppressive medication as needed during regular lab review

Job Responsibility

Evaluation: Reassesses patients at appropriate intervals following interventions. Notifies physician as necessary to modify interventions. Adjusts nursing care to meet patient's emerging and changing needs.
Report Skills: Gives and receives report using Bedside Shift Report including family participation. Provides an accurate and comprehensive report to the oncoming shift to help establish continuity of patient care. Receives report from off-going shift and delegates tasks as needed to provide continuity of care according to coworker's experience, abilities and the patient's acuity.
Environment & Patient Safety: Maintains patient and work environment to maximize safe working space. Promptly removes all used and soiled equipment, linen, etc. to appropriate location. Incorporates national and organizational goals to improve quality, patient safety and satisfaction. Reports all patient safety and hazard concerns immediately.

Uses knowledge of the database to extract data and generate required reports and queries for statistical analyses in varying formats as requested
Analyzes transplant information for governmental agencies, physicians, managed care organizations, research studies, etc. by entering raw data into a computer, organizing and manipulating data, collecting data, consulting with team members, and generating graphs and charts
Prepares reports, presentations, and other documents as instructed
Maintains transplant and cellular therapy database
Reviews and audits database on a regular basis to ensure accuracy of data
