Baptist Medical Center is looking for an Care Manager to join our team in Jacksonville, FL. This is a Full-Time days opportunity located at Baptist Metro Square campus.

Job Responsibility:

  • High level screening, assessing and planning for an identified high risk group of patients in the ER, Critical Care and other services as needed
  • Collaborate with the attending, ancillary, professional and technical staff to coordinate patient care
  • Screen patient for admissions or discharge utilizing Utilization Review criteria
  • Participate in oversight of resources in critical care and emergency services area
  • Assess patients for Hospital at Home eligibility and/or Medical Bridge Clinic

Requirements:

  • 3-5 Bedside RN experience
  • Bachelor of Science in Nursing required
  • Basic Life Support (BLS) required
  • Active Unrestricted Florida RN License required
  • Case Management Experience

  • Recognize, diagnose, and manage both acute and chronic medical conditions in order to prevent destabilization and readmissions
  • Provide after-hours and weekend call support, shared with other in-state providers
  • Facilitate and conduct goals of care and advance care planning discussions with patients and families
  • Assess and evaluate family/caregiver needs and limitations
