The Transition of Care Coach is responsible for care coordination of members experiencing significant changes in health status resulting from emergency department visits, inpatient, skilled nursing, or rehabilitative stays. The role ensures seamless transitions to next care settings and facilitates post-discharge goal attainment through comprehensive discharge planning, medication management, benefit education, and clinical assistance.
