Transfer Coordinator

• Coordinates all processes, support, communication and documentation involved in transferring patients from one facility to another facility, as well as support physician consults
• Recognize and uphold all regulatory requirements, while simultaneously updating key stakeholders of timelines
• Accurately enters all required data into EPIC database
• Coordinates with Flight for Life for patient transports

• High School Diploma/GED, required
• Certified Healthcare Access Associate (CHAA) successfully completed within 18 months of employment
• One to two years of direct clinical experience in Emergency Department/Critical Care Unit (such as Critical Care Tech EMT-B etc.), or one to two years of full time direct administrative healthcare experience (such as Nursing Unit Secretary or Patient Access Representative) or one to two years Transfer Center experience
• Working knowledge of medical terminology, excellent computer skills, customer service, ability to multi-task efficiently and effectively
• Physical Requirements-Sedentary work -(prolonged periods of sitting and exert up to 10lbs force occasionally)
