An exciting opportunity currently exists for an experienced Grade 2 Allied Health Professional to join the Emergency Department (ED) Care Coordination team at Western Health on a fixed-term, part-time basis (0.8 EFT) working across Western Health’s multiple ED’s on 7-day rotating roster (including both AM and PM shifts). This role delivers Emergency Department Care Coordination within a transdisciplinary model of care, supporting timely, patient-centred assessment, intervention, and discharge planning for patients presenting to the Emergency Department, with a focus on reducing re-presentation.
Job Responsibility:
Assessment and care planning in collaboration with patients, their carers and members of the wider Emergency Department team
Referral, liaison, and advocacy to appropriate community services where indicated
Supporting continuity of care across the acute and community interface
Assisting with achieving ED targets and KPIs
Supervision of Students Grade 1s and Allied Health Assistants
Requirements:
A minimum of two years’ clinical experience in an Emergency Department setting
Experience in a relevant acute and/or community setting
Strong discharging planning and problem-solving skills
Knowledge of appropriate community services and supports
Current registration with AHPRA (where applicable)
Excellent communication, organisational, and time management skills
Current Victorian Driver's Licence
