Job Description:

Team Leader position ensuring Dementia Support Australia service is tailored to individual clients and delivers on key performance indicators. Supporting people living with dementia transitioning from hospital settings back to their homes or into residential care.

Job Responsibility:

  • Provide leadership to multidisciplinary team across multiple locations
  • Work as part of integrated team for nationally consistent approach
  • Develop culture of empowerment, mentorship and support
  • Oversight of individual and team to ensure highest quality service delivery
  • Working in partnership with other Team Leaders
  • Contribute to administrative, financial, risk and quality reporting goals
  • Provide direct consultation and support to clients
  • Ensure timely allocation of incoming referrals
  • Provide regular reporting to Regional Manager
  • Recruit appropriate staff
  • Key stakeholder management with residential aged care, home care and hospital settings
  • Effectively manage customer complaints or challenging situations
  • Work with Regional Manager to address incidents and feedback
  • Improving own knowledge and skills
  • Oversee office environment safety and maintenance
  • Oversee local use and maintenance of fleet vehicles
  • Responsible for delegated approvals for brokerage and travel

Requirements:

  • Commitment to engage and align with HammondCare’s Christian Mission, Motivation and Mission in Action
  • Tertiary qualifications in Nursing/Allied Health or health-related degree
  • Demonstrated experience and knowledge in supporting older people and carers living with dementia
  • Ability to manage and support a team of skilled professionals across multiple sites and locations
  • Ability to work on multiple tasks and projects simultaneously
  • High level communication and interpersonal skills
  • Strong planning and organising skills with ability to prioritise tasks
  • Strong analytical skills and attention to detail
  • Good customer management skills
  • Strong administrative and computer skills
  • Willingness to travel intra and interstate

Nice to have:

  • Eligibility for registration as a Health Practitioner in Australia
  • Evidence of study and/or qualifications in dementia
