Job Responsibility:
Support the home management team in delivering high-quality care
Lead in the person-centred care planning process by implementing, monitoring, and undertaking monthly evaluations
Complete care plan audits, develop action plans, and be responsible for completing the action plans
Assist in the smooth transition of any discharges from the home, including documentation and medication
Keep the residents' next of kin fully informed, where appropriate
Lead with monitoring the well-being of each resident
Ensure changing needs are met and written records are amended to reflect any variation
Practice safe systems of work across a range of tasks and moving and handling of residents
Assess risks and have due regard for personal safety and the safety of residents, visitors, and staff
Requirements:
NVQ 3 or equivalent in care for care homes
Two years’ experience working in a senior position with the resident group
Ability to communicate effectively both verbally and in writing
Ability to lead staff and act responsibly in the absence of the manager
