Join CatholicCare Wollongong as Team Leader – Care Partners, where you will lead, coach and develop a high-performing team while maintaining a small caseload of your own. We are recruiting a Team Leader to provide operational leadership across our Care Partner team in the Aged and Disability Services program. Based within the Catholic Diocese of Wollongong, this role drives the quality and consistency of care across the team while keeping the program on track against its strategic goals.
Job Responsibility
Lead, supervise and coach the Care Partner team across regional areas, building a culture of accountability, inclusion and continuous improvement
Manage team and individual performance including appraisals, recruitment, training and professional development
Maintain a small caseload, providing direct case management and supporting clients to achieve their goals within funded budgets
Monitor program compliance against Aged Care Quality Standards, legislative requirements, funding obligations and organisational KPIs
Support the attraction and retention of new clients and provide a consistent entry point for new Home Care Package enquiries
Assess and address risk, escalate issues appropriately and report regularly to the Executive Manager on team outcomes
Requirements
Minimum 3 years' experience leading a team within the aged care sector
Relevant tertiary qualifications in aged care, health, community services or business management (or Certificate IV in Leadership and Management, or working towards)
Sound knowledge of the My Aged Care system and community-based aged care service delivery
Demonstrated complex case management experience
Strong coaching, relationship-building and stakeholder engagement skills
Understanding of contractual, legislative and governance requirements including the new Aged Care Reforms
Nice to have
Experience supporting a team through service reform or organisational change
Familiarity with rostering systems and cost-effective service scheduling
Experience with business development or client growth activities in a community care setting
