Job Description:

Baptist Health is looking for a Social Services Team Lead at our Baptist Beaches campus. This is a Full Time Days opportunity. The Social Services Team Lead will assess psychosocial needs and provide services that enhance patient functioning, emotional well-being and continued access to services and resources in the community. Provide discharge planning, arrange home health and DME, assist with placement/transfers, provide referrals to community services.
