Job Responsibility:

  • Assess psychosocial needs and provide services that enhance patient functioning, emotional well-being and continued access to services and resources in the community
  • Provide discharge planning, arrange home health and DME, assist with placement/transfers, provide referrals to community services
  • Social work resource for team members and care coordination program
  • Provide guidance through challenging situations
  • Promote problem-solving/critical thinking skills among team members
  • Assist with interviewing and hiring staff members
  • Coordinating orientation and training for newly hired staff

Requirements:

  • Master's Degree
  • 1-2 years Social Work Experience
  • Case Management Experience
