Pathways to Good Health is a multidisciplinary health care service for children and young people who have entered out of home care. The service aims to provide these children and young people with health management plans that meet their unique needs in a timely manner.

Job Responsibility:

Assisting the Pathways to Good Health team to support Culturally and Linguistically Diverse (CALD) and Aboriginal and Torres Strait Islander children within Child Protection system to navigate and access health care

Requirements:

  • Have knowledge and familiarisation with assessments for children and young people aged 0-18 years
  • Demonstrate the ability to work effectively and sensitively with children and families with Child Protection involvement
  • Respect for and understanding of the child/young person’s right to informed choice, independence and determination of services required and delivered
  • Ability to apply principles of confidentially, privacy and maintain confident communication
  • Can work independently, be self-motivated and want to bring your knowledge base to the wider team
