Job Description:

Brook Street Social Care is seeking compassionate and motivated Support Workers to join our Day Services team in Preston. This role involves providing high-quality care and support to individuals with learning disabilities, helping them to live fulfilling lives with greater independence, choice, and control.

Job Responsibility:

  • Support individuals to build confidence, self-esteem, and communication skills
  • create a welcoming, inclusive, and respectful environment
  • encourage individuals to make decisions about their lives
  • develop positive working relationships with families, carers, and professionals
  • assist individuals in accessing activities per person-centred plans
  • plan and deliver structured sessions tailored to individual needs
  • maintain clear records of participation and progress
  • attend review and planning meetings
  • support individuals in age-appropriate community activities
  • provide personal care and health monitoring
  • provide physical support for wheelchair users
  • complete required training
  • work collaboratively with families, carers, volunteers, and professionals
  • act as an advocate for individuals
  • promote best practice in safeguarding, health and safety, and mental capacity
  • engage in regular supervision and team meetings
  • work in line with organisational policies
