Support Worker

• Supporting individuals with complex needs, including mental health, physical health, and substance misuse issues
• Assessing and managing risks to ensure safety and well-being
• Delivering high-quality care, support, and housing management in line with organisational policies
• Working collaboratively with partner agencies to provide holistic, tailored support
• Developing and implementing client-centred support and risk management plans, aiming for suitable move-on options within six weeks
• Building positive relationships, motivating progress, and encouraging recovery
• Managing challenging situations effectively, ensuring safety, and reporting safeguarding concerns
• Maintaining accurate case records, updating IT systems, and producing reports
• Participating in rota shifts, including evenings, weekends, and bank holidays, to ensure continuous support
• Assisting clients with housing benefit claims and tenancy-related administrative tasks

• Understanding of the needs of homeless people and experience working face-to-face support
• Experience supporting individuals with complex needs
• Knowledge of trauma-informed care and move-on options
• Excellent communication skills, adaptable to diverse clients and stakeholders
• Strong decision-making skills and the ability to respond effectively in challenging situations
• IT proficiency, including Microsoft Office and database management
• Ability to work independently and as part of a team with a proactive attitude
• Relevant qualifications or certifications in health, social care, or related fields are desirable
• Commitment to safeguarding, equality, and diversity
