Job Description:

We are seeking a passionate and caring Support Worker for our Supported Living Service in Pontefract, WF8. This post is for 37.5hrs per week. The Service is a unique bungalow with 4 bedrooms. The tenants share the communal lounge and kitchen. We provide 1:1 support for 4 tenants. All tenants have an individual support plans, ensuring they are supported in a person-centred way and are promoted to make own choices. Each person has their own individual care package of support.

Job Responsibility:

  • Providing 1:1 support for 4 tenants
  • Implementing individual support plans in a person-centred way
  • Promoting personalised choice and options regarding independent living, wellbeing and independence
  • Supporting tenants in individual activities
  • Working closely with families and significant others within the person's circle of care
  • Promoting equality, diversity, and following professional boundaries
  • Ensuring tenants have the 'Best life Possible'
  • Meeting tenants' care needs

Requirements:

  • Passionate and caring
  • Can be fully flexible around day shifts
  • Ideally hold full UK Driving Licence
  • Highly reliable and motivated
  • Flexible to meet service and tenant needs
  • Able to have positive relationships with families and individuals in circles of support
  • Committed to specific shift patterns (07.30 to 15.00, 14.00 to 21.30, sleeping night 14.45 to 22.15 finishing at 07.30 the following day, waking night 21.30 to 07.30)
  • Committed to complete all required training
