Job Description:

As a Support Worker, support people with disabilities to live independently, connect with the community, and pursue their interests. Assist in personal care, daily living activities, and social inclusion. Collaborate with multidisciplinary teams and empower individuals to achieve their goals.

Job Responsibility:

Provide person-centred support across home and community settings
assist with daily living activities including high personal care
share interests and hobbies to enrich participant experiences
support social inclusion and community participation
collaborate with a multidisciplinary team to deliver individualised support plans
empower people to achieve their goals and live life on their terms
provide support with water sports including swimming
