Job Description

Support is uniquely tailored to each individual, ensuring they have choice and control over the planning and delivery of their support. You will support them to reach their potential by providing both physical and emotional support, along with encouraging them to develop positive relationships and achieve their goals.

Job Responsibility

  • Support individuals with personal care, medication, cooking, and household tasks
  • Encourage social interaction with friends, family, and community events and promote independence
  • Support with making and attending health care appointments
  • Communicate with medical professionals and follow support plans and risk assessments
  • Maintain clear and accurate records of support provided
  • Communicate effectively with the team, family members
  • Work independently and as part of a supportive team
  • Encouraging physical activity – including walking and playing games

Requirements

  • Must be able to work as part of a team and lone working
  • take initiative and problem-solving skills
  • Enhanced PVG required
