We are seeking a compassionate, resilient and experienced specialist support worker to provide exceptional care for an individual with complex Autism and learning difficulties who displays self in-injurious and challenging behaviours. You must be able to communicate effectively using a person-centred communication passport whilst delivering a high level of support to achieve an enriched and fulfilled life through implementing strategies to manage their dynamic needs effectively. The individual will be living independently, so we are recruiting a committed team who can provide 24 hour, wrap around support that covers all aspects of their life.

Provide specialist support to enrich my future, support me with activities of daily living and accessing the community while respecting my dignity and promoting independence
Be a positive advocate to enable me to make informed choices and act in best interest when I am unable to do so
Be consistent, reliable and pro-active in implementing my positive behaviour support plans with a calm and understanding can-do attitude
Build a trusting and positive relationship with me fostering a sense of security, comfort and confidence
Work as part of the team, delivering exceptional person-centred care, following my individualised care-plan
Ensure the environment is safe and supportive, regularly assessing risks and taking proactive steps to reduce harm
Observe and document changes in behaviour, health, or emotional well-being, providing detailed reports to the service Manager
Maintain high standards and professional boundaries while maintaining relations with my family and the wider healthcare professionals
Requirements:
Must have minimum of 3 years LD/Autism support worker experience or equivalent
Be physically fit to engage in physical intervention and to encourage an active and healthy lifestyle
Can Drive legally in the UK and have own transport Driving Licence (MUST)
