Fern is a service supporting 6 gentlemen with a variation of needs. The role involves supporting them to learn new skills, attend activities, achieve personal goals, and bring out the best in them through active support.
Job Responsibility:
Support service users with administration of medication
Support with personal care needs and nutritional needs
Support with making and attending medical appointments
Support food shopping and individual’s personal shopping needs
Support with laundry duties and cooking and cleaning
Support with finances and other administrative tasks
Support with moving and handling requirements and using hoists
Support with social activities and accessing community-based services
Offer emotional support to service users and act as a positive role model
Write daily notes and complete relevant forms
Complete daily observation sheets
Implement policies and procedures of the organisation
Work under supervision to implement individual service user’s care plans
Work under supervision to prepare individual assessments on service users
Keep service users safe and report any changes or health concerns
Attend training, participate in supervision sessions and attend staff meetings
