Job Description

Fern is a service supporting 6 gentlemen with a variation of needs. The role involves supporting them to learn new skills, attend activities, achieve personal goals, and bring out the best in them through active support.

Job Responsibility

  • Support service users with administration of medication
  • Support with personal care needs and nutritional needs
  • Support with making and attending medical appointments
  • Support food shopping and individual’s personal shopping needs
  • Support with laundry duties and cooking and cleaning
  • Support with finances and other administrative tasks
  • Support with moving and handling requirements and using hoists
  • Support with social activities and accessing community-based services
  • Offer emotional support to service users and act as a positive role model
  • Write daily notes and complete relevant forms
  • Complete daily observation sheets
  • Implement policies and procedures of the organisation
  • Work under supervision to implement individual service user’s care plans
  • Work under supervision to prepare individual assessments on service users
  • Keep service users safe and report any changes or health concerns
  • Attend training, participate in supervision sessions and attend staff meetings
