Job Description:

Our client is a locally owned and leading NDIS provider in the top end and is seeking a passionate specialist support coordinator to contribute to their mission. These services include Coordination of Supports, Specialist Support Coordination, Specialist Behaviour Supports, and Music Therapy.

Job Responsibility:

  • As a Specialist Support Coordinator, you'll work closely with individuals facing complex care and medical needs throughout their lives, helping them overcome barriers to implementing their NDIS plans
  • You'll build vital relationships with participants, their families, disability service organizations, clinical staff, and various government departments like health, education, communities, and justice
  • Your role is all about fostering a collaborative approach, ensuring support is person-centered and delivered efficiently
  • You'll also be instrumental in establishing best practice care, providing training and support, and intervening to achieve positive, sustainable outcomes for participants, especially after a crisis

Requirements:

  • Demonstrated well-developed oral, written and interpersonal communication skills
  • Demonstrated ability to work well independently and cooperatively as a member of a team
  • Established ability to listen actively and demonstrate empathy
  • Well-developed organisational skills, the ability to manage time effectively and prioritise tasks.
  • Working with Children card
  • NDIS Worker Screening
  • Police check (within 6 months)
  • current Drivers Licence and roadworthy vehicle
  • Computer literacy & efficiency in Microsoft Office
  • A bachelor's degree in Social Work, Occupational Therapy, Physiotherapy, Psychology, Nursing or other Allied Health
  • Current Australian driver's licence and roadworthy vehicle
  • Working with Children (Ochre) Card
  • National Police Clearance
  • First Aid Certificate
  • NDIS Worker Screening
  • The right to work in Australia
