Our client is a locally owned and leading NDIS provider in the top end and is seeking a passionate specialist support coordinator to contribute to their mission. These services include Coordination of Supports, Specialist Support Coordination, Specialist Behaviour Supports, and Music Therapy.
Job Responsibility:
As a Specialist Support Coordinator, you'll work closely with individuals facing complex care and medical needs throughout their lives, helping them overcome barriers to implementing their NDIS plans
You'll build vital relationships with participants, their families, disability service organizations, clinical staff, and various government departments like health, education, communities, and justice
Your role is all about fostering a collaborative approach, ensuring support is person-centered and delivered efficiently
You'll also be instrumental in establishing best practice care, providing training and support, and intervening to achieve positive, sustainable outcomes for participants, especially after a crisis
Requirements:
Demonstrated well-developed oral, written and interpersonal communication skills
Demonstrated ability to work well independently and cooperatively as a member of a team
Established ability to listen actively and demonstrate empathy
Well-developed organisational skills, the ability to manage time effectively and prioritise tasks.

A bachelor's degree in Social Work, Occupational Therapy, Physiotherapy, Psychology, Nursing or other Allied Health
