Support Coordinator

• Lead a supported Living service providing support for 7 tenants
• Provide personalised support to people with learning disabilities and complex mental health needs living in their own homes
• Direct support, staff deployment and supervision
• Joint working with families and other agencies
• Coordinate staff team to ensure service users enjoy the highest quality of life whilst promoting independence
• Work flexibly to include evenings and weekends as part of a rota to meet individual’s needs

• Previous experience of working with people experiencing mental health problems and learning disabilities either in a community or supported living setting
• Skills in positive engagement
• Knowledge of positive behavioral support
• Excellent verbal and written communication skills
• Understanding of the principles of the recovery model
• Good working knowledge of mental health diagnosis and legislation
• Ability to confidently communicate and participate in support planning with members of multi-disciplinary teams
• Excellent leadership, planning and coordination skills
• Minimum of Level 3 Diploma in Health and Social qualification
• Must be located in and eligible to work within the UK

Relevant social care degree or qualified social worker
