Job Description:

We are looking for a dedicated Support at Home Care Partner to provide exceptional care and services that enable older individuals to remain safely at home. You will play a vital role in developing, implementing, and monitoring personalized support plans. This role is about one hour from Warrnambool.

Job Responsibility:

  • Work within a legislative framework to ensure compliance with the Support at Home Program
  • Guide Health Care Workers to deliver safe and timely care services
  • Actively identify and manage safety risks for participants
  • Collaborate effectively within a team and ensure high-quality service delivery
  • Communicate openly and participate in multi-disciplinary team meetings

Requirements:

  • Care Management Qualification (or currently working towards)
  • Cert 4 Individual Support- with relevent expereince
  • Current registration with the Nursing and Midwifery Board of Australia or similar
  • Valid Victorian Driver's License
  • Current Police Check
  • Current AHPRA registration (if required)
  • Influenza Vaccine (Current Year)
  • Strong time management and prioritization skills
  • Experience in individualized support within a person-centered framework
  • Ability to identify and manage participant safety risks effectively

Nice to have:

Familiarity with software such as e-Tools, TRAKCare, RiskMan, and Microsoft Excel
