Job Description

The Senior Social Worker will facilitate the ongoing development and implementation of a Children’s Health Home Care Management program and provide leadership to a team of Children’s Health Home Team Leads and Care Managers for the purpose of implementing the Golisano Children’s Hospital at Strong Health Home Care Management Program.

Job Responsibility

  • Facilitate the ongoing development and implementation of a Children’s Health Home Care Management program
  • Provide leadership to a team of Children’s Health Home Team Leads and Care Managers
  • Assure comprehensive care management services are provided in accordance with established requirements and protocols
  • Responsible for recruitment, training, supervision and evaluation of team leads and oversight of care managers
  • Facilitate integration of program services with pediatric and behavioral health practices
  • Develop collaborative relationships with community-based programs and services
  • Perform quality assurance activities, program evaluation, and reporting

Requirements

  • 2 years of professional social work practice
  • MSW degree from an accredited school of social work
  • Licensed MSW in New York State
  • Significant clinical experience with a pediatric population in a medical, behavioral health, or child welfare setting

Nice to have

  • Supervisory and program evaluation experience
  • Knowledge of health care and Health Home delivery systems
