This role is based within an inpatient hospital setting. The standard work schedule is Monday to Friday, 8:00 a.m. to 4:30 p.m. Acts as Supervisor of Healthy Community Programs for the Palliative Care department.
Job Responsibility:
Responds to palliative care referrals within 24-72 hours based on established referral criteria
Assesses, plans, and implements appropriate interventions related to the psychosocial needs of Palliative Care patients and their families
Utilizes training to address end-of-life issues throughout the hospital
Documents assessments and other relevant data related to interventions in the patients’ medical records
Provides individualized services to patients and families throughout hospitalization
Stays informed about community resources and assists patients and families in utilizing appropriate resources
Acts as a liaison between family members, physicians, and hospital staff in complex cases involving multiple physicians
Manages critical care cases in the ICU
Educates hospital staff and physicians about Palliative Care and promotes the program
Serves as an integral member of the multidisciplinary care team and regularly attends Palliative Care meetings
Provides support and supervision to interns, medical students, and Critical Care Academy nurses, including orientation and training for new staff
Requirements:
Master’s degree in family therapy, counseling social work or related discipline
Minimum two (2) years’ experience in field of social work, counseling or family therapy or completion of 1 year internship in Palliative Care
Prefer medical experience or working with families with life limiting or life-threatening illnesses
