Professional Counselor license issued by the state of Wisconsin
Marriage and Family Therapist license issued by the state of Wisconsin
Advanced Practice Social Worker license
Social Worker, Clinical license issued by the state of Wisconsin
Master's Degree in Social Work
Master's Degree in Psychology, Clinical
Master's Degree in Psychology, Educational
Master's Degree in Psychology, Counseling or related field
Typically requires 2 years of experience in providing therapy in behavioral health
Some positions may be restricted to individuals with Master's Degree in Social Work and be a Licensed Clinical Social Worker upon hire

Job Responsibility

Provides individual and group therapeutic services in the inpatient level of care
Functions as a member of the interdisciplinary treatment team by reviewing assessment data, case management, intake, and other discipline data
Ensures goals identified in the treatment plan are clinically appropriate to the stated problems, development focused, measurable and achievable within current anticipated lengths of stay and updates treatment plans as appropriate
Performs patient psychosocial assessments to determine appropriate interventions and care plan, goal determining and treatment plan altering as the case warrants
Contacts families, where appropriate for collateral information and discharge planning
Facilitates family meetings, communicates schedule and outcomes with members of the interdisciplinary team
May be responsible for monitoring and evaluating the appropriateness of social services provided
Performs peer to peer chart audits consistent with regulatory requirements
Works cooperatively with other professional and non-professional staff to identify and resolve problems in the service delivery systems, ultimately improving the quality of patient care
Maintains or exceeds clinical productivity standards established by the Manager

Preferred: Bachelor’s degree or 5 years relevant experience
Preferred to have a minimum of two (2) years serving persons with mental health and substance use disorders
A valid Wisconsin-Substance Abuse Counselor or Clinical Substance Abuse Counselor (SAC, CSAC) and in good standing with the state or application for credential within 90 days of hire
A valid Certified Peer Specialist Certification (CPS) or must obtain certification within 90 days of hire
Must hold a current and active driver’s license and be in good standing
Current American Heart Association CPR certification or American Red Cross Professional Rescue or certification to be obtained within 30 days of hire. Bi-annual re-certification is required
Formal training in crisis prevention and crisis intervention is required within sixty (60) days of date of hire. Annual recertification is required
First aid training is required within ninety (90) days of employment

Job Responsibility

Provide SUD support services consistent with training, credentialing, and scope of practice
Facilitate individual and group recovery-focused sessions
Support patients in developing and completing Recovery Maintenance Plans
Implement components of individualized treatment plans and reinforce program philosophy
Monitor patient behavior, intervene as appropriate, and promote a safe, therapeutic environment
Document patient progress, behaviors, and responses to interventions in a timely manner
Serve as a case manager as assigned through the interdisciplinary team
Coordinate discharge and continuing care planning in collaboration with therapists and team members
Initiate and maintain collateral and community contacts to support ongoing recovery
Educate patients on community supports, sponsorship, mentorship, and recovery resources

Understand and implement daily patient activities collaborating with staff
Monitor behavior and intervene to ensure dignity, learning and well-being of all patients, and reinforce behavior though interactions based on the program philosophy
Provide and document SUD group facilitation
Use knowledge and skills to collect, review and document information regarding the patient’s status
May complete clinical reports such as Social Services Admission Assessment, admission/discharge ASAM assessments
