Job Description

The stroke center coordinator is a leader who is responsible for the organization of stroke patient care with an emphasis on evidence based clinical practice guidelines and outcomes management. With the stroke medical director, the coordinator leads the development and implementation of program goals and collaborates with physicians and staff to promote care delivery that assures efficiency, positive clinical outcomes, and cost effectiveness.

Job Responsibility

  • Chair the entity stroke committee in collaboration with medical leadership
  • Oversee the care of stroke patients in collaboration with other members of the health care team to meet all care standards per UH stroke clinical practice guidelines and regulatory requirements
  • Respond to stroke alerts to provide support to staff and expedite patient care
  • Develop and provide education related to the care of stroke patients to physicians, nursing staff, ancillary staff, support staff, and EMS
  • Abstract data to meet all regulatory requirements for stroke certification and to provide feedback to the interdisciplinary team
  • Analyze data and quality outcomes to identify trends and implement process improvement projects to improve the quality of care provided
  • Participate in the system stroke committee meetings and integrate system initiatives at the entity level
  • Facilitate community outreach risk screenings and educational programs related to stroke
  • Performs other duties as assigned
  • Complies with all policies and standards
  • Must abide by all requirements to safely and securely maintain Protected Health Information (PHI) for our patients

Requirements

  • BSN Bachelor’s Degree in Nursing (Required)
  • 3+ years of clinical nursing experience, preferably in areas of ED, ICU or neurology/neurosurgery (Required)
  • Previous experience with The Joint Commission Stroke Center standards and data collection (Required)
  • Registered Nurse (RN), Ohio and/or Multi State Compact License (Required Upon Hire)
  • Basic Life Support (BLS) for Healthcare Providers (Required)
  • Excellent interpersonal communication skills, both written and verbal, as well as strong organizational skills. (Required proficiency)
  • Strong problem solving, teaching, and assessment skills. (Required proficiency)
  • Proven ability to lead teams through program development and implementation (Required proficiency)
  • Computer skills in all office applications with proficiency in MS Word, Excel, and PowerPoint (Required proficiency)
  • Ability to function independently with minimal supervision and meet goals and deadlines (Required proficiency)
