Job Description

Single Homeless Project has an opportunity for a Specialist Health Lead to join our experienced and committed teams based in Camden. You will join us on a full-time, permanent basis. In return, you will receive a competitive salary starting at £34,127.11 and rising incrementally to £36,947.48 per annum.

Job Responsibility

  • Shape how health support is delivered within Camden Housing First service
  • Complete health assessments and design personal health support plans
  • Lead multidisciplinary meetings
  • Open doors to essential services
  • Map local services
  • Influence partnerships and drive forward inclusive healthcare approaches

Requirements

  • Knowledge (or a real willingness to learn) about common health issues and the barriers to care faced by people experiencing homelessness
  • Know how to build trust and connect with clients who may feel wary of services
  • Understand local health systems and services
  • Experience or awareness of working with survivors of domestic abuse
  • Confident working independently and as part of a team
  • Strong skills in support planning, safeguarding, record keeping, data and reporting, and advocating for clients facing multiple disadvantage
