At Noah’s Ark you'll have opportunity to build on your experience and be trained to a more specialist level. We’re expanding our team and are looking for more Specialist Carers to join our charity. You’ll be joining an award-winning, certified Outstanding team, who do all they can to empower children and families to experience as much life as possible.

Job Responsibility

  • Provide life-changing care for babies, children and young people with life-limiting or life-threatening conditions
  • Work in collaboration with family, multidisciplinary teams and other Noah’s Ark colleagues
  • Put the experience of children and families at the heart of all you do
  • Get to know what’s important to each child you support
  • Focus on what children can do, rather than their limitations
  • Support children to experience things they may never have thought possible
  • Provide care at our state-of-the art building, The Ark, or out in the community
  • Allow parents and carers to take some time for themselves
  • Be a senior carer in our team and pass on your skills and experience by supporting and training junior colleagues
  • Actively lead care planning reviews, projects and meetings
  • Provide end-of-life care
  • Be a guiding reassuring presence for families
  • Work with other healthcare professionals to create as much comfort as possible

Requirements

  • Experience caring for babies, children and young people with complex health needs
  • Competent in a range of complex nursing skills
  • Understanding the power of play and enjoy creating experiences and activities
  • Share core values of kindness, excellence, and courage
  • Ability to work autonomously and deliver care alone in the community
  • Value of collaboration and work closely with team members, other healthcare professionals and families
  • Creativity and play central to approach to care
  • Ability to build long-term relationships
  • Understand and adhere to policies and procedures at all times
  • Competency in clinical skills, working under the delegation of a registered children’s nurse
  • Available to work a variety of shifts, including days, evenings, nights and weekends
  • Full UK driving license and access to your own car

  • Undertake comprehensive patient assessment to include physical, psychological, social and spiritual elements of care using best evidence based practice in urological diseases
  • Use the outcomes of patient assessment to develop and implement plans of care/case management in conjunction with the consultant and the patient, family and/or carer as appropriate
  • Monitor and evaluate the patient’s response to treatment and amend the plan of care accordingly in conjunction with the Consultant and patient, family and/or carer as appropriate
  • Make alterations in the management of patient’s condition in collaboration with the Consultant and the patient in line with agreed pathways and policies, procedures, protocols and guidelines (PPPG’s)
  • Accept appropriate referrals from Consultant colleagues
  • Co-ordinate investigations, treatment therapies and patient follow-up
  • Communicate with patients, family and /or carer as appropriate, to assess patient’s needs and provide relevant support, information, education, advice and counselling as required
  • Where appropriate work collaboratively with MDT colleagues across Primary and Secondary Care to provide a seamless service delivery to the patient, family and/or carer as appropriate
  • Participate in medication reconciliation taking cognisance of poly-pharmacy and support medical and pharmacy staff with medication reviews and medication management
