Job Description:

Are you passionate about improving wellbeing? We are looking for an organised, self-motivated and confident individual to join our Social Prescribing team, helping adult and child patients to access support that will help them lead more independent, resilient and healthier lives.

Job Responsibility:

Help patients and families connect with health and community services
Deliver personalised support to patients
Independently manage new referrals from health professionals and a caseload
Prioritise need
Recognise when to refer on to specialist services
Communicate at all levels across health and social care sectors

Requirements:

Experience of working with adults or children
Background in medical or social care
Experience of delivering health and wellbeing support to clients or groups
Robust knowledge of confidentiality, data protection, safeguarding and boundaries
Car driver essential
Right to work in the UK
