Conduct comprehensive psychosocial assessments for dialysis patients and families and develop individualized care plans in collaboration with the interdisciplinary renal team
Provide ongoing counseling and crisis intervention to support adjustment to chronic illness, ESRD, loss of independence, grief, and end‑of‑life concerns
Identify and address barriers to dialysis adherence, including transportation, housing instability, insurance challenges, financial hardship, and caregiver support needs
Coordinate care with the interdisciplinary renal team, including nephrologists, dialysis nurses and technicians, and case management staff
Educate and connect patients and families to community, governmental, and healthcare resources, providing advocacy and care coordination
Assist patients with insurance acquisition and maintenance to support uninterrupted dialysis treatment
Complete required documentation and regulatory reporting, including CMS Form 2728 and CrownWeb activities, in accordance with CMS and Renal Network standards
Requirements:
Master’s Degree in Social Work (MSW) with active Illinois LSW licensure required
LCSW preferred (or eligibility in progress)
Experience in hospital‑based social work (inpatient and/or outpatient), with dialysis or renal care strongly preferred
Demonstrated experience providing psychosocial assessment, counseling, and crisis intervention for medically complex patients
Working knowledge of Medicare/Medicaid, care coordination, and social determinants of health impacting safety‑net populations
