Provides professional social work services to patients and their families. Collaborates with hospital staff and community social services resources relative to the psycho-social problems of patients. Participates in formulating procedures and policies of the Division of Social Service. Participates in educational and research programs, including the orientation and training of new staff and students.
Job Responsibility:
Performs psychosocial assessments of patients and families
Provides social work service including crisis intervention, supportive counseling, problem-solving, information and referral
Collaborates with other clinic care providers in development and implementation of interdisciplinary care plans
Coordinates referrals to and acts as liaison to community agencies
Provides assessment and intervention services for children and adults suspected of being victims of abuse, neglect and/or domestic violence
Handles risk management issues
Documents social work activity in the electronic medical record
Provides social work coverage to other clinical units or programs during co-workers absence
Completes required in-service training programs and ongoing professional development activities
Attends all appropriate Social Work Division meetings
Supervises social work students (MSW or BSW)
Orients and provides ongoing peer support to new social work staff
Requirements:
MSW from an accredited social work graduate program
Must obtain New York State license within eighteen months of employment
Prior clinical experience in a medical setting preferred
Supervisory experience preferred
Familiar with medical social work and community resources preferred
