Job Description:

As a Social Worker in the Adult Locality Team, you will support adults with a range of complex needs, including physical disabilities, mental health issues, and learning disabilities. You will work to assess their needs, provide appropriate care planning, and facilitate access to services that promote independence, dignity, and well-being. This role focuses on delivering person-centered support while adhering to statutory requirements under the Care Act 2014 and other relevant legislation.

Job Responsibility:

  • Conduct thorough assessments of adults referred to the locality team, identifying their needs, risks, and strengths
  • Develop and implement personalised care and support plans that promote well-being and independence
  • Carry out mental capacity assessments in accordance with the Mental Capacity Act 2005
  • Make applications for Deprivation of Liberty Safeguards (DoLS) where appropriate, ensuring legal compliance and protecting individuals' rights
  • Handle urgent and complex cases with sensitivity and efficiency, providing crisis intervention and support as needed
  • Maintain accurate, clear, and timely records of assessments, interventions, and care plans, ensuring all documentation is in line with council policies and legal standards.

Requirements:

  • Relevant social work qualification
  • 2 experience in adult social work, including assessment and care planning.
