Job Description:

An excellent opportunity to join a Locality Team in Bath & North East Somerset as a Social Worker conducting Care Act assessments. Responsibilities include needs assessments, care planning, safeguarding, and working within a multi-disciplinary team.

Job Responsibility:

Undertaking comprehensive needs assessments under the Care Act 2014 for adults requiring social care support
developing person-centred care and support plans in collaboration with individuals, families, and carers
conducting Mental Capacity Act (MCA) assessments and acting in individuals' best interests where appropriate
working effectively within a multi-disciplinary team
managing a diverse caseload with varying levels of complexity
safeguarding vulnerable adults and responding to safeguarding concerns
providing advice and information to service users and their families

Requirements:

Qualified Social Worker with current registration with Social Work England
demonstrable experience in adult social care, particularly with a focus on Care Act assessments
in-depth knowledge of the Care Act 2014, Mental Capacity Act 2005, and other relevant adult social care legislation
excellent assessment, analytical, and risk management skills
strong communication and interpersonal abilities
ability to work autonomously and as part of a supportive team
