We are seeking a compassionate and proactive Social Worker to join a Hospital Discharge team. This is a vital role, ensuring seamless and safe transitions for patients leaving hospital, and providing crucial support to both individuals and their families. Job Responsibility

  • Conduct comprehensive social work assessments to identify patients' needs and develop personalised discharge plans
  • Collaborate effectively with multidisciplinary teams, including medical staff, nurses, therapists, and community services
  • Arrange appropriate care packages, including home care, equipment, and financial support
  • Provide information, advice, and advocacy to patients and their families
  • Safeguard vulnerable adults and children, adhering to relevant legislation and policies
  • Maintain accurate records and contribute to service development
  • Work to prevent delayed discharges and promote patient independence

Requirements

  • Qualified Social Worker
  • SSSC registered as a Social Worker
  • Car driver
  • At least 3 years hospital discharge experience
  • BA in social work
