The social worker is responsible for providing clinical services to patients admitted to their assigned units and their families. The social worker serves as a member of the interdisciplinary health care team on their assigned unit and in other inpatient and outpatient areas as assigned, primarily in Wilmot Cancer Institute. Social Worker serves as a liaison to health and social service agencies in the community.
Job Responsibility:
Provides clinical services to patients admitted to assigned units and their families
Serves as a member of the interdisciplinary health care team
Coordinates discharge planning for patients determined to be at psychosocial risk
Serves as a resource consultant to patients and families
Facilitates referrals to community agencies and hospital resources
Provides social work interventions including risk screening, psychosocial assessments, crisis intervention, supportive counseling, grief and bereavement counseling
Handles risk management issues
Documents social work activity in electronic medical record
Provides social work coverage to other clinical areas as requested
Completes required in-service training and professional development
Attends Social Work Division meetings
Supervises social work students
Orients and provides peer support to new social work staff
Requirements:
MSW from an accredited social work graduate program
Must obtain New York State license within eighteen months of employment
Prior clinical experience in a medical setting and supervisory experience (preferred)
Familiar with medical social work and community resources (preferred)
